A smoke alarm going off is not damage. It is the alarm doing its job.

A smoke alarm going off for eleven months is a different object entirely. Nothing about the sound changed. What changed is that nobody ever came to say the kitchen is fine.

Your body does not charge you for the alarm. It charges you for leaving it on.

That is the whole subject of this piece, and it reframes the question most people ask. "How do I lower cortisol" usually gets answered with a list of things to add. The mechanism says something different. You are not missing an ingredient. You are missing an ending.

What Does Chronic Stress Actually Do to Cortisol?

Chronic stress does not simply raise cortisol like a dial. It degrades the timing and the shutoff, so the hormone stays present when your body should have finished with it.

Cortisol: the primary glucocorticoid hormone in humans, released by the adrenal glands and coordinating the body's response to demand.

HPA axis: the hypothalamic-pituitary-adrenal axis, the three-part chain that releases cortisol and then regulates itself through negative feedback.

The system is self-limiting by design. Cortisol goes out, and cortisol coming back acts on the same circuit to shut off the signal that released it. When that feedback works, the loop closes on its own.

Sleep is where most people break it first, and the effect is faster than most people expect. Rachel Leproult and colleagues tracked plasma cortisol across a 32-hour window in young men under normal sleep, partial deprivation and total deprivation. After partial and total sleep deprivation, evening cortisol levels were 37% and 45% higher on the second day, and the onset of the quiet period of cortisol secretion was delayed by at least an hour. Their conclusion was that even partial acute sleep loss delays the HPA axis recovery and likely involves an alteration in negative glucocorticoid feedback.

Read that last part again. One short night did not just add cortisol. It interfered with the mechanism that takes cortisol away.

Why Doesn't Modern Stress Ever Resolve?

Because most of what activates you now has no ending built into it. The event you are responding to is not in the room, cannot be handled, and refreshes.

The cleanest demonstration of this is uncomfortable. Two to four weeks after the Boston Marathon bombing, Alison Holman, Dana Garfin and Roxane Cohen Silver surveyed 4,675 adults nationally, with representative subsamples from Boston and New York. Adjusting for pre-bombing mental health collected prospectively, demographics and prior collective stress exposure, six or more daily hours of bombing-related media exposure in the week afterward was associated with higher acute stress than direct exposure to the bombing itself. Controlling for pre-bombing television habits did not change the result.

Holman's own explanation of why is the part worth keeping. She described repeated exposure as making it feel like the event continues and has its own presence in your life, and said prolonged media exposure can turn what was an acute experience into a chronic form of stress.

Honest limit, stated plainly: this is a survey, not an experiment. It cannot rule out that people already running hot chose to watch more coverage. But the comparison group is hard to wave off. A screen carrying zero physical risk tracked with more stress response than standing near an actual bomb.

How Does High Cortisol Affect Performance and Thinking?

It moves control of your behavior out of the region that deliberates and into the region that reacts, and the shift is structural when the exposure is sustained.

Prefrontal cortex: the region behind your forehead responsible for working memory, planning, judgment and self-control.

Amy Arnsten's 2009 review in Nature Reviews Neuroscience pulled together decades of animal and human work. Even mild uncontrollable stress rapidly impairs prefrontal function. Sustained stress goes further, retracting the dendrites on prefrontal neurons while the same branching structures expand on neurons in the amygdala (Arnsten, 2009).

Sonia Lupien, Bruce McEwen, Megan Gunnar and Christine Heim made the broader case in the same journal the same year (Lupien et al., 2009). Chronic exposure to stress hormones affects brain structures involved in cognition, with the specific effects depending on the timing and duration of exposure.

This is the practical translation of an abstract hormone. It is why month four of something hard feels different from week one. It is why you have a shorter fuse than you used to and cannot point to the day it changed. The circuit that reacts gained surface area. The circuit that thinks lost some.

What Actually Closes the Stress Loop?

Two mechanisms, and they turn out to be the same system.

Mechanism one: the fast shutoff. Cortisol's own rapid negative feedback at the hypothalamus is not a purely genomic process. Nathan Evanson, Jeffrey Tasker, Matthew Hill, Cecilia Hillard and James Herman infused glucocorticoids directly into the paraventricular nucleus in rats and got rapid inhibition of the stress response (Evanson et al., 2010, Endocrinology). That fast feedback response was blocked by a CB1 receptor antagonist, indicating the shutoff requires local endocannabinoid release.

Endocannabinoid system: a signaling network built around CB1 and CB2 receptors and the molecules the body makes to activate them, including anandamide.

Mechanism two: extinction. Interrupting an alarm is not the same as resolving it. Resolution is a specific kind of learning.

Extinction: unlearning a fear response once the cue stops predicting anything bad.

Giovanni Marsicano and colleagues showed in 2002 in Nature that mice lacking CB1 receptors had strongly impaired short-term and long-term extinction in fear conditioning while memory acquisition and consolidation were unaffected, and that presenting the tone during extinction trials raised endocannabinoid levels in the basolateral amygdala (Marsicano et al., 2002).

The system was not needed to learn the fear. It was needed to let it go.

That translates to people. A common variant of the FAAH gene degrades the enzyme that breaks anandamide down, leaving carriers with more of it circulating. Ozge Gunduz-Cinar and colleagues found carriers showed faster habituation of amygdala reactivity to threat. Iva Dincheva and colleagues then built a knock-in mouse carrying the same human mutation and found reduced FAAH expression that selectively enhanced fronto-amygdala connectivity and fear extinction learning and decreased anxiety-like behaviors across both species (Dincheva et al., 2015, Nature Communications).

The same circuit. The one stress degrades over months is the one this system supports.

Where this gets mixed is the drug version, and it deserves both halves. In healthy adults, a FAAH inhibitor raised anandamide roughly tenfold and was associated with better recall of fear extinction memory 24 hours later, blunted autonomic stress reactivity, and protection against stress-induced negative affect (Mayo et al., 2020, Biological Psychiatry). But in a 100-person randomized trial pairing the same class of drug with exposure-based therapy, symptoms improved over time in both arms with no effect of FAAH inhibition, despite confirmation that the drug did raise anandamide levels (Neuropsychopharmacology, 2025).

So: the mechanism is demonstrated across mice, genetics and human imaging. A reliable pharmacological shortcut is not. Both of those are true and I am not going to publish only the flattering one.
